Appointments & Cancellation

For your convenience, appointments can be scheduled by calling our office at 480-393-0575. As a courtesy to other patients, we ask that appointment cancellations be made at least 24 hours in advance by calling our office.

What to Bring to Your Appointment

Please bring the following items to your office visits:

  • Insurance card(s)
  • Driver’s license or other photo identification
  • List of your current medications, supplements, and dosages
  • Any pertinent medical records or test results
  • Insurance Co-payment (We accept Visa, MasterCard, Discover, checks and cash)
  • Worker’s Compensation Information (If Applicable)
  • Referral forms (If required by insurance)

We currently accept many major health insurance plans. Please check our list of Contracted Health Insurance Plans to see if your plan is covered. Because the list may change without notice, we encourage patients to contact our office to verify coverage before scheduling at 480-393-0575.
